Monogram Health and Memorial Hermann Launch Joint Venture

Partnership will expand access to high-quality in-home care for patients in Southeast Texas with multiple chronic conditions

NASHVILLE, Tenn. and HOUSTON, March 4, 2025 /PRNewswire/ — Monogram Health—the nation’s leading value-based multispecialty provider of in-home, evidence-based care for patients with multiple chronic conditions—and Memorial Hermann Health System, a nonprofit health system committed to creating healthier Houston communities, have launched services to provide care for commercial and Medicare Advantage members of Memorial Hermann Health Plan, the insurance division of the Houston-based health system.

This joint venture between Monogram and Memorial Hermann brings together Monogram’s expertise in providing multispecialty in-home care with Memorial Hermann’s high-value care network that includes its experienced, affiliated physicians and over 270 care delivery sites across Greater Houston. The launch of services for Memorial Hermann Health Plan members aims to deliver holistic, patient-centered care that addresses the complex needs of individuals living with chronic kidney disease and its causes, including diabetes, high blood pressure and metabolic disorders.
